Body Wellness's treatments will vary for each client depending on their needs and goals. Due to this the cost per session can vary between clients and sessions. Below is a basic breakdown of costs.

Payment and Insurance Billing
Payment is expected on the date service is rendered for non insurance billing patients.
Massage therapy benefits vary between individual carriers and plans. Body Wellness LLC is not currently In-Network with any insurance companies. Some carriers allow you to self submit claims for out of network providers. You are responsible for inquiring with your provider in regards to that. I am happy to provide you with any needed documentation for you to self submit.
I can bill Aflac accident insurance, Work-mans Comp, Auto insurance, and some other employment benefit providers. Please inform me before your appointment if you plan on billing an insurance company, and bring your insurance card to the first appointment.
Full payment for the session is required within 30 days for missed or canceled appointments unless 24 hrs notice is given. My schedule is tight and I have a waiting list for people who want appointments, so I can often fill a spot if proper notice is given.
Body Wellness accepts all major credit cards, cash, and local checks.
To expedite the Initial Treatment session, it is best for you to print and fill out this health history and intake form. There are 2 sides of this form. For filing purposes, please print them on only 1 sheet of paper.
